Interactive Online Forms

The interactive online forms including ‘Request a Prescription’, ‘Book an Appointment’, and ‘New Patients’ are filled out and submitted at the user’s own discretion. By submitting this information, you are agreeing to provide the partners of Ross Medical Practice with your personal information. Ross Medical Practice assures all users that this information is initially stored securely in a password protected encrypted email server (Proton Mail) before being deleted therein and transferred to the practice consultation software (Health One). Data will be deleted from the email server in a timely manner once processed in keeping with General Data Protection Regulation  (GDPR) and the principle of confidentiality which governs our work as medical practitioners. New patient requests are subject to review by the practice partners to ensure the practice is capable of meeting your medical needs at the time of submitting your request to join Ross Medical Practice. Any submitted information for patients who are not accepted as New Patients will be deleted promptly as per GDPR regulation.

By submitting these interactive online forms, you are agreeing to permit Ross Medical Practice to contact you. Users may alternatively contact Ross Medical Practice by phone or in person, or they may not contact Ross Medical Practice if they so wish. Your information will not be made available to any third parties in keeping with the principle of confidentiality which governs our practice as medical practitioners.

Limitation of liability

Under no circumstances (including but not limited to any act or omission on the part of Ross Medical Practice) will Ross Medical Practice be liable for any indirect, incidental, special and/or consequential damages or loss of profits whatsoever which result from any use or access of, or any inability to use or access, the Site or any Content. You expressly acknowledge and agree that Ross Medical Practice does not exert control over users of the Site (including individuals referred to on the Site as guests and experts) and is not liable either for their opinions or their behaviour including any information and/or advice and any defamatory statements or offensive conduct.
